The following forms are required for each band member.

Forms must be submitted via the Rank One application.

1. FBISD Athletic Participation Signature Page.
2. FBISD Emergency Contact Form
3. UIL Forms Signature Page.
4. UIL Physical Form needs to be Uploaded here.

Simply go to https://fortbendisd.rankone.com and follow the instructions in the Parent Portal.
